I thought grinding my teeth at night was just something stressed people did. Then I saw the data, and four months later, I no longer sleep with a mouthguard thanks to DTR treatment at Bliss Dental.
An Illuminating Experience: DTR at Bliss Dental, North London
I have slept with a mouth guard for longer than I care to admit. The hard plastic kind, custom-made, that sits on my bedside table like a small transparent horseshoe and which I reach for automatically each night before the lights go off. To not wear it is to lie awake hyper-aware of my own jaw, conscious of a low, persistent tension I can’t quite unknot. My dentist made it for me after noticing the wear on my teeth. It helped. But it never, exactly, fixed anything. And, frankly, it is not the most attractive bedside companion.
So when someone mentioned they had undergone something called DTR – Disclusion Time Reduction at Bliss Dental in North London and described the process as “like someone finally turning the volume down in your jaw,” I booked an initial consultation without any particular expectations. What followed over the course of the appointments has been one of the more genuinely illuminating experiences I have had in a dental chair.
The Start of the Story: Noticing the Problem
The practice is run by Dr Jeremy Bliss, who has a manner that is equal parts methodical and reassuring. Before anything else, he took a full history: not just dental, but the whole picture. Stress levels, sleep quality, neck issues, the works. He wasn’t surprised to hear that I rated my stress at around a ten out of ten, or that jaw ache tended to track my worst weeks. “Most people don’t connect the two,” he said. “They assume it’s just tension. But the teeth are usually where the story starts.”
The assessment was unlike any dental appointment I had had before. Rather than the familiar carbon paper pressed between teeth and a dentist squinting at the marks, Dr Bliss used two technologies in tandem: T-Scan, a computerised bite-force device with a thin sensor you bite down on, and EMG, electromyography, which places small sensors on the jaw and temples to measure muscle activity in real time.
The results were a revelation. A 3D model of my bite appeared on-screen as I clenched and slid my jaw side to side, colour-coded by pressure and timing. Roughly 60% of the force from a full clench was loading onto the left side of my mouth. When I moved my jaw to the right, the back teeth on the left stayed in contact for too long. This delay, Dr Bliss explained, is called prolonged disclusion time, and it was triggering visible spasms in my left temporal muscle. I could see them on the screen. My jaw was, quite literally, fighting itself.
The ideal disclusion time, the speed at which the back teeth should separate when the jaw moves sideways, is around half a second or less. Mine was well over that. The problem wasn’t vague or psychological. It was measurable, visible, and addressable.
Mouth Guards Only Treat Symptoms
This was the part that shifted something in my thinking. I had always assumed my mouth guard was solving the problem or was the only solution. Dr Bliss gently clarified that it wasn’t: it was managing the consequences. “A night guard protects your teeth from the force of grinding,” he said, “but it doesn’t change why you’re grinding, or how your muscles are behaving. You can wear it for twenty years, and your muscles will still be hyperactive.”
In patients with high stress, strong jaw muscles, and prolonged disclusion time, the muscles essentially never fully switch off. The back teeth act as brakes during lateral movement, keeping everything engaged in a state of semi-constant tension. Over time, this contributes to headaches, neck stiffness, fatigue, and disturbed sleep, symptoms that many of us, myself included, file under modern life. Or hormones. Or both.
What is DTR?
Disclusion Time Reduction (DTR) is a treatment that reduces the time the back teeth remain in contact during lateral and protrusive jaw movements. Using T-Scan and surface EMG, the dentist identifies specific interferences and makes minimal, targeted enamel adjustments, typically less than 1mm, to allow the canine teeth to guide the jaw more quickly.
The goal is a disclusion time of 0.5 seconds or less, which allows the chewing muscles to decompress and breaks the spasm-tension cycle driving chronic bruxism and jaw-related headaches.
A Rundown of the Treatment
I returned a few weeks later for the first real adjustment. We reviewed the scan data; Dr Bliss walked me through the exact teeth causing the interference, explaining how a single cusp catching fractionally too long can be enough to keep a muscle in chronic contraction. Then, using both T-Scan and articulating paper to confirm contacts in real time, he began making changes.
These are not dramatic interventions. No drilling, no anaesthetic. The process involves minimal, controlled smoothing of specific enamel surfaces, the high spots and drag points that the computer has identified. Each adjustment takes seconds. Each is immediately re-scanned. An interference that had been registering at over 50% of bite force involvement on the right side was reduced to around 16% in that single session. I could feel the difference at once, a looseness in the way my jaw moved, as if something that had been snagging was now sliding freely.
“The teeth have been compressed slightly over the years, and they’ll rebound as the muscles start to relax.”
“You might find your bite feels strange for a day or two,” Dr Bliss told me. “The teeth have been compressed slightly over the years, and they’ll rebound as the muscles start to relax.” He was right. For the first forty-eight hours, my jaw felt oddly unfamiliar, not uncomfortable, but newly spacious. Like stretching a muscle you had held tight for so long, you had forgotten it could lengthen.
A Newfound Awareness of the Jaw
One unexpected consequence of the treatment is what happens to your consciousness of your own bite. Before DTR, I barely thought about how my teeth came together. Now I notice everything. I catch myself mid-conversation, registering whether my jaw is clenching and trying to make sure I don’t clench unconsciously.
The bite, it turns out, is exquisitely sensitive to everything going on in the rest of your life. Stress is the most obvious factor: on a high-pressure day, the masseter muscles tighten reflexively, and you can feel the quality of your occlusion shift. Sleep is another. A broken night correlates directly with increased morning jaw stiffness. Even posture plays a role; hours hunched at a desk alter the relationship between upper and lower jaw in ways that, once you are paying attention, become hard to ignore.
In some ways, this awareness is uncomfortable. You notice things you had previously tuned out. But Dr Bliss framed it well: “The discomfort is information. It’s your body telling you something it’s been trying to communicate for years; you’re just finally listening.” The hyperawareness, he suggested, settles as the bite stabilises and the muscles learn to rest. He was right about that, too.
A Continuous Process
DTR is not a treatment you complete in two or three sittings and forget about. It is iterative by design: as the muscles decompress and the teeth rebound from years of compression, the bite shifts again, sometimes by fractions of a millimetre, but enough to warrant further refinement. By the fourth appointment, we were sitting at roughly 55% left, 45% right, considerably closer to even than at the start. New contacts on the right had emerged, in the canine and premolar region, which Dr Bliss worked through methodically, each one identified by data, each adjustment verified by the scan before I left the chair.
That iterative quality requires a certain trust. You have to be willing to sit with a bite that is in transition, knowing the destination is worth the journey. The data helps: it is easier to accept an unfamiliar sensation when you can see precisely why it is there and what is being done about it.
I started this treatment in February. It is now four months on, and I am sleeping without my mouth guard. After years of reaching for it automatically before the lights went off, it simply stopped being necessary. More than that: my jaw feels different. There is a sense of it sitting slightly further forward than before, the muscles no longer pulling everything back and inward into the old pattern of tension. When I wake now, I am aware, in a way I never was before, that my teeth are not clenched together. They are apart. Resting. It is a small thing, perhaps. It does not feel small.
The low-grade tension that I had come to think of as simply how my head felt, the tightness behind the eyes, the faint ache along the jaw by early afternoon, has largely lifted. Not entirely, and not every day. Stress is still stress, and a difficult week still registers in my jaw. But the baseline has shifted. The biting, where it occurs, feels lighter. The muscles are no longer starting from a position of chronic contraction.
What strikes me most, reflecting on all of it, is how much I had simply normalised. The guard, the tension, the headaches, all of it had become background noise that I had stopped questioning. It took someone looking at the actual data to make it legible. And now that I can hear what my jaw has been trying to say, I find I am rather glad to have started listening.
How to Know if DTR is Suitable for You
DTR is not appropriate for every case of grinding or jaw tension; an initial consultation with diagnostic scanning is required to establish whether your bite pattern would respond to this approach. The cost of a full course sits between £2,500 and £3,000, reflecting both the technology involved and the number of appointments typically needed.
But if you wear a night guard out of necessity rather than choice, if you have headaches that cluster around your temples and jaw, or if you have simply accepted a background level of facial tension as the price of a busy life, it may be worth having someone look at the data. You might find, as I did, that the problem is considerably more specific and considerably more solvable than you had assumed. It just requires a dentist willing to look beyond the paper.
